Why is this happening? What can I do to resolve?

In FME Server 2016 + I am unable to request a license through the web interface with the following error: "Failed to retrieve license request connection info."

The "Failed to retrieve license request connection info." can occur for a number of reasons including: if the machine you are licensing is not connected to the internet, if our licensing service is down, if there are any firewall/ port issues not allowing the request through, or if on a cloud hosted environment the url to the licensing service is not whitelisted.
In any of these cases, please use the manual/offline .json method and send the resultant file to codes@safe.com.
