For the Writer Parcel, I chenaged the format for DWG 3D:

What do you mean by "change the format for DWG 3D"?

The Output - I inserted a new format in yoir Workspace.

First of all nice profile picture :)

Secondly I have had a look at your workspace and tried it myself.

The extended data is written to the output as expected, see the screenshot below.

BTW you dont have the add the default application name to the template, you can edit that value in the advanced writer parameters. I have used FME as the value for the parameter.
