I haven't tried this myself yet but there's some helpful information from our help doc on Leaders under Feature Representation for AutoCAD DWG. I believe you'd be able to set the attributes required for it in the AttributeManager.
Maybe someone else in the Community will be able to share their experiences working with autocad leaders?
Set the autocad entity to autocad_leader then define the path type, straight, spline. You can feed a polyline in to a leader. I use the offsetter to offset points radially from the place where I want a label then I create a polyline from these points and attach text and leader.
