@Richard Webb Hi there, I'm sorry to hear you've encountered this issue! It does sound like it's very much related to a Known Issue we have in at least FME 2022.2.6 and FME 2023.0.0.1. What version and build of FME Form are you currently using?
For reference, here's the FME Community Known Issue notice that was just recently released, which does sound very much like the behavior you're seeing.
@Richard Webb Thanks for the additional info. And what version of ArcGIS Enterprise Portal are you trying to connect to? Would it happen to be version 11.1, released roughly April 20, 2023?
If it is indeed version 11.1 of Esri ArcGIS Enterprise Portal that you're trying to connect to, it looks like the issue has been resolved in our latest release of FME Form (version 2023.0.1). I would suggest giving FME Form 2023.0.1 a try with your Portal connection and seeing if this resolves the 500 error for you!
Please let me know how this test goes. If you see positive results with FME Form 2023.0.1 and your ArcGIS Enterprise Portal 11.1, we may have to add some FME builds to the Known Issue notice I linked above. Thank you!

Sorry I only just got back to this after EOFY reporting madness.
I can confirm build 23338 is happily talking to ArcGIS Enterprise Portal 11.1
