Short answer: yes.
Long answer: yes, but it depends on the data i/o. FME Could is certainly capable of real-time operation. There's an overview here: https://knowledge.safe.com/articles/1484/fme-server-real-time-data-processing.html (it's about FME Server, but most if not all of this would apply to FME Cloud as well). If you can give some more information on what you'd like to do we can come up with a better answer.

As Hans say, yes, there's no reason why this can't be done with FME Server/Cloud. Exactly how would depend on your data and the method of transmission.
