it needs a keyboard shortcut. Having just right clicked it and duplicated it serveral hundred times I can verify this.

I'm using 2013 Build 13263 and It's not working/ doesnt show up on the menu.
It is possible that Itay uses SP1 as I know that it will introduce a few more hotkeys. It is due out this month, so you can either wait a few more weeks or you can download it now as a beta.
On the other hand, you could simply select all the reader feature types (either with the selection lasso or by Shift-clicking), then right-click one of them and select "Duplicate on writer". This will perform the operation on all the selected feature types.
Tested on FME2013 buil 13264 (but I think this has been the case for a long time).

it seems we are using the same build no. of FME, but Ctrl-D does nothing for me and it is not listed under the feature type context menu. Where did you find the reference to this hotkey?
Also, a fanout will not always make sense in this scenario. You are perhaps thinking about a Merged feature type, or am I missing something?

FME 2013 builds 13262 and 13264 both duplicate a Transformer and a Writer (FFS) for me!

I get the same thing, Ctrl-D duplicates either a transformer or a writer feature type. But it does not do "Duplicate on writer" from a reader feature type, at least not in this build version.
I guess that's where the confusion lies :-)

The cntrl+d was introduced with FME2013, I use it for feature type/transformer duplication since the release.
Naturally using the fan out on the writer will only result in writer feature tyes and may not be useful in this situation. Some more details about the writer format type can be useful.
For a 'writer' fan out, I find usually find dynamic writing to be more suitable.
