In a workspace I'm writing to a gdb (api), this works fine. If i use this workspace again it results in an error: FME can't write to the GDB. Parameter "overwrite existing database" = "YES"
MDB gives no troubles

I seemed to have the issue when writing to a new GDB from time to time (with that parameter "overwrite existing database" set to yes)
Now and then I have an issue with the "Template File Geodatabse" parameter. I have to change its value from <Unused> to <not set> if I want the process to work:
