Many of our ArcSDE connections are OS Authentication and not username/password. How can I connect to the SDE instance with ArcSDEQuerier? It seems that the ESRI Geodatabase reader/writer uses a connection file but the Querier doesn't.

Currently, the ArcSDE querier does not support OS Authentication. There is a PR for this functionality, that I will attach this question to. This posting will be updated with any changes to the PR regarding this functionality.
Alternatively, you could create an idea requesting the functionality, this would allow different users to upvote this idea and better increase the community awareness for the developers. The Ideas forum can be found here: https://knowledge.safe.com/content/idea/list.html
